Course Objectives

In the rapidly changing landscape of ageing, health and social care are often intertwined and plays a pivotal role in enabling citizens to age in place. The Specialist Diploma in Integrating Health & Social Care goes beyond general knowledge by equipping participants with the skills and strategies needed to address the complex challenges of managing health and social issues. Participants will have confidence to design and implement innovative programs, services, and interventions tailored to the unique needs of individuals and communities.
The proposed course is designed for professionals and mid-careerists seeking to advance their careers and contribute to the holistic well-being of individuals and communities.

Course Description

The Specialist Diploma in Integrating Health & Social Care (SDIS) is a part-time CET course, consisting of the following modules:
1) Social Prescribing & Asset-Based Community Development (45hrs)
2) Promoting Positive Health Behaviours to Support Patients and Caregivers (45hrs)
3) Case Management in Health & Social Care (45hrs)
4) Programme Management in the Community (45hrs)
5) Empowering Community through Volunteer Management (45hrs)
6) Group Dynamics and Facilitation (45hrs)

Empowering Community through Volunteer Management (NP-SECTVM)
Participants in the module will be introduced to key principles and structures in volunteer management. Participants will develop knowledge in recruiting, selecting, and onboarding volunteers, along with implementing strategies for retention through recognition and community-building. The module further covers risk management, legal compliance, and promotes diversity and inclusion within volunteer programs. Participants will also learn methods to measure volunteer impact on organizational goals leading to optimized volunteer engagement and organizational effectiveness.
Group Dynamics & Facilitation (NP-SGDF)
This module explores key principles and theories of group dynamics, emphasizing skills in forming groups, fostering effective communication, and conflict management. Participants will develop leadership abilities and apply cultural competence to promote diversity and inclusion within groups, focusing on building cohesion, trust and adapting approaches based on evolving group needs via positive interactions.
Programme Management in the Community (NP-SPMC)
The module is designed to equip participants with essential skills in programme management such as needs assessment, programme frameworks and evaluation processes in the community. Participants will gain confidence in designing, implementing, and evaluating programs by combining problem-solving, decision-making, resource allocation considerations to optimize project efficiency via practical case studies.
Post-Diploma Certificate in Holistic Approach in Health and Social Well-Being
Case Management in Health & Social Care (NP-SCMHSC)
This module is designed to provide learners with the foundational knowledge and practical skills in case management. It covers key principles such as assessment, planning, coordination, and evaluation, which are essential for effective patient/ client support in health and social care settings. In addition to theoretical learning, learners will also engage in practicum where they have the opportunity to apply their knowledge and skills in a real-world setting. Facilitated under the guidance of experienced professionals, learners will gain hands-on experience and practical insights into the role of case management in health and social services.
Promoting Positive Health Behaviours to Support Patients and Caregivers (NP-SPPHBSPC)
This module provides learners with the essential insights and practical techniques to effectively support patients/ clients in their health journeys. Learners will explore the principles of person-centred care and learn how to facilitate meaningful conversations that inspire positive behaviour change. Through interactive sessions and case studies, learners will discover strategies to build trust with their patients/ clients, set achievable goals, and help them navigate challenges effectively.
Social Prescribing & Asset-Based Community Development (NP-SSPABCD)
In this module, learners will explore the impact of Social Prescribing and Asset-Based Community Development. They will gain the essential knowledge and skills to empower individuals and communities, promoting better health outcomes and strengthening social bonds. Through practical insights and real-world case studies, learners will also grasp the significance of health and social care integration and discover how community-based initiatives can enhance the overall well-being of patients/ clients.
